What forms should I fill out before my appointment?

Do I need a referral?

Many insurance companies prohibit physicians from treating a patient without an appropriate referral. If your plan requires a referral, you will need to have it called into Pima Heart prior to an appointment or brought with you at the time of your visit. Please contact our office for assistance with your specific insurance plan requirements.

What is your cancellation policy?

If you are unable to keep a scheduled appointment, please notify our office 24 hours prior to your appointment. If you may be late for a scheduled appointment, please contact the office as soon as possible.

Do you have parking?

Each office has convenient parking available.

How do I make an appointment?

Patients are seen by appointment Monday through Friday. Contact the office most convenient for you and a Pima Heart Staff Member will schedule an appointment. Established patients can request an appointment via our patient portal.

What are your hours of operation?

8:30 a.m. – 5:00 p.m.

Do you have a list of participating insurance carriers?

We accept most major insurance carriers. Please contact us to check your specific policy or carrier.

If I don’t have insurance – how will I be billed? Do you support a payment plan?

Patients without insurance are expected to pay in full when services are rendered. Pima Heart does support a payment plan, which is discussed on an individual basis.

Do you accept credit card payments?

We accept Visa and MasterCard.
